Licensed Vocational Nurse
Minimum Requirements
Graduate of an accredited practical nurse or vocational nursing program.
One year nursing experience, or at least six months experience if concurrently working in a Hospital or Home Health Setting.
Community health/hospice or medical/surgical experience is preferred.
Currently licensed as an LPN/LVN in the state of California.
Complies with accepted professional standards and practice.
Demonstrates good verbal and written communication skills.
Demonstrates good organization skills.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Understands hospice philosophy. Also understands needs of the terminally ill.

Essential Job Functions and Responsibilities
- Provide direct patient care as defined in the State Nurse Practice Act.
- Implement current nursing practice following a patient assessment and an approved Plan of Care initiated by the Registered Nurse.
- Provide accurate and timely documentation of patient services in accordance with the Plan of Care.
- Assess and provide patient/family/caregiver education and information pertinent to the patient’s diagnosis and Plan of Care.
- Participate in the coordination of hospice services, appropriately reporting the identified needs to hospice Interdisciplinary Group members. Examples include hospice aide, OT, PT, MSW, ST, Dietitian, and Clinical Supervisor.
- Use equipment and supplies effectively and efficiently.
- Provide appropriate pain/symptom management. Evaluate the patient’s response to treatments/medications.
- Participate in personal, professional growth and development, as well as in the organization's quality management program.
- Perform other duties as assigned by the RN Case Manager.
